If you want to send us a saliva sample for testing, then please get the following:
1. One small bubble wrap envelope
2. One small zip-lock plastic bag
3. Approximately 8cm x 8cm (3" x 3") of white paper towel.
4. Click here to display a form to be filled in. Print out the form and fill in relevant information (please print):
Now do the following:
1. Write the following address on the envelope:

2. Saturate the tissue paper by chewing on it and then spit it into the zip-lock bag.
3. Seal the zip-lock bag and put it into the envelope together with the filled in form (and bank draft if applicable) and send by air-mail.
We will respond with a report and an instruction of what in our opinion you should do to improve your health.
